Jose Ayala, 47, of Simi Valley died of severe head trauma when he became trapped in an industrial die casting machine at 9 a.m. Oct. 16 at Vista Lighting Company in Simi Valley.

According to police reports, the Ventura County Fire Department performed heavy rescue to extricate the victim from the machinery. Ayala was in full cardiac arrest when he was removed. Fire department personnel performed CPR, but Ayala was pronounced dead at the scene.

An investigation is being conducted.
