SCAN Health Plan, one of the premier health plans serving seniors throughout Southern California, has received approval to provide healthcare benefits to Medicare-eligible individuals in Moorpark and Simi Valley.

SCAN’s provider partner in Moorpark and Simi Valley is Regal Medical Group, which will offer members more than a dozen primary care physicians from which to choose. The participating hospital is Simi Valley Hospital.

For a $30 monthly premium with no deductibles and small copayments, SCAN members can receive all of the basic benefits covered under Medicare, including doctor visits, hospital care, emergency/urgent care, ambulance service, home healthcare, skilled nursing facility care and chiropractic/podiatry care.

Beyond these core benefits, SCAN members have coverage for brand and generic prescription drug coverage, routine eye and hearing exams, eyewear, hearing aids and transportation to the doctor’s office.

Another benefit is the 24-hour nurse advice line SCAN OnCall™, in-home assistance services, and access to emergency care worldwide.
